Description

If you need a refresher, reread the instructions on how to respond to the Discussion areas in this course using the Questions, Quotations, and Talking Points (QQQTP) Approach.

The discussion for module 3 is related to equity and equality in healthcare. For this Discussion:

Read one of these two articles:

Coombe, C., Schulz, A., Guluma, L., Allen, A., Gray, C., Brakefield-Caldwell, W., Guzman, J., Lewis, T., Reyes, A., Rowe, Z., Pappas, L., & Israel, B. (2020). Enhancing capacity of community–academic partnerships to achieve health equity: Results from the CBPR partnership academy. Health Promotion Practice, 21(4), 552–563.

Roy, B., Kiefe, C., Jacobs, D., Goff, D., Lloyd-Jones, D., Shikany, J., Reis, J., Gordon-Larsen, P., & Lewis, C. (2020). Education, race/ethnicity, and causes of premature mortality among middle-aged adults in 4 US urban communities: Results from CARDIA, 1985-2017. American Journal of Public Health (1971), 110(4), 530–536.

While reading, take notes so you can gather information for your post. For example, as a question comes to mind be sure to write it down.  Same for the quotation, when one comes along that strikes you, write it down.

When you are done reading, take time to reflect on that reading material so that you can formulate a post to the discussion area by Thursday of the first week of the module that includes the following:

post a question that invites analysis, synthesis, or evaluation of the reading material or makes connections between the readings and previous discussions or readings.  Your classmates will answer this question.

provide a quotation, one that is especially pertinent or relevant to the main points of the reading and one that will prompt significant discussion;

post two (2) Talking points that you think with initiate discussion amongst your classmates. These points should show that you have thought seriously and critically about the readings.  These should let your classmates "see inside your head" as to what you think, accept, reject about the reading material.  Your 2 talking points should cover different topics from the readings.  The purpose of formulating and posting these is to help you organize your thoughts and focus your ideas.

Now, let's get the discussion going - you need to respond to at least 2 of your classmates’ initial discussion posts by either answering their questions or discussing their quotation or one of their talking points.  At least one of your responses should be based on the article you did not address in your initial post. In addition, you need to monitor your own discussion thread and respond by dialoguing with peers and/or provide a summary at the end of the module.  Remember, this is a discussion area and it is polite to respond when you receive a post. Be sure to enter the discussion regularly throughout the entire discussion. Do not wait until the last few days of the module to become active.

Question:       Having worked both a hospital with vast cultural diversity and one with very little cultural diversity, I believe it is difficult for staff to truly understand how subtleties in cultural approach to health care can affect outcomes.  As with everything in life, practice makes perfect.  It is one thing to attend a seminar on a subject and entirely different to actually practice a subject.  What are some methods of how staff, who are open and willing to learn how to better serve culturally diverse patients but work in areas with very little diversity, practice in the field classroom taught behaviors?

Quotation:     Abrishami (2018) noted the following:

“…national standards have been established to promote patient-centered care that reduces or eliminates health disparities in the U.S. population. The importance of cultural competence and awareness of patient diversity has been the main theme in national standards. Therefore, it is imperative that health care educators emphasize the importance of cultural competency training and education in their programs.”

It is important that these standards were developed on a national level so that a more inclusive cultural competency model could be developed.  Providing a locally developed training model would most likely exclude a large portion of the population, especially in areas where there is very little cultural diversity.

Talking Points:

1.  Although, in my opinion inadvertent, I believe that many health care providers have reduced the effectiveness of providing optimal health care to culturally diverse patients by not understanding or speaking to their patient’s cultural beliefs.  As discussed by Fayth Parks in her TedxAugusta discussion How culture connects to healing and recovery (2015), the Culturally-Based Healing Beliefs System regards Spirituality, Power-of-Words, Ritual and Dreams as highly influential in the efficacy of positive health care experiences and results.

2.  The Bennet Model of cultural competency, as discussed by Abrishami (2018) seems to be an outdated view of society.  As part of the Bennet Model, the health care provider refuses to accept that patients have cultural diversity.  In my opinion, this may have been true prior to the civil rights movements where segregation was a common and accepted practice but would be very difficult to continue in today’s day and age.  Social media allows people to connect in ways never imagined and to discuss issues with service.  If a health care provider were to openly discriminate against culturally diverse patients, the current wave of “cancel culture” would quickly drive that provider out of business.
